Please print the Health Information and Office Policies forms and fill out prior to your scheduled appointment.

If you are unable to print and fill out, then please arrive 10-15 minutes before your scheduled time.  We want to make sure you get the entire 60-90 minutes of "table" time!

There are some illnesses, diseases and injuries that require a physician, physical therapist, or chiropractor's permission/prescription before we can begin treatment.  If you have a question about your specific case, please call to discuss prior to your arrival.
